Youtube added an "advanced bitrate" option to the very top of the quality settings for premium that pixelates and destroys already-pixelated or blurry video when it's on. You have to manually go down one quality setting now on those videos to actually get real HD video.

Exactly what I was looking for, after being frustrated with YouTube's constant video quality decrement whenever I open a video. I'm actually surprised that someone thought of a solution for this! And I like the continuous monitoring at specified intervals- because one might open a new video, or you never know when YT will decrease the quality again! Kudos to the developers for keeping it so subtle!
